Uncheck the "Variable 'From' field" option

You have enabled 'Variable From field' option in Simfatic Forms.

This is an override to set the 'From' field of your form submission emails to be the email submitted in the form.

Most email servers will reject the email and you may not receive form submission emails because of this.

Solution

Go to Simfatic Forms menu item Tools->Options and uncheck the 'Variable From field' option.
Re-upload your form
